In order for your child to start school, please be aware that certain health requirements must be met.  The New York State Education Law requires that school children in grades K, 1st, 3rd and 7th and students new to the district must have a physical examination.  If you plan to have your family doctor examine your child, please ask him to complete the attached Annual Physical Exam Form and return it to the school nurse by October 1st of the current school year.  After this date, the school physician reserves the legal right to make a physical appraisal of all pupils for whom we have no report.

If your child has been seen by your family dentist, or is currently under treatment, please have him fill out the Annual Dental Exam Form.

Your child will be excluded from school if he/she does not have the required immunizations.  Public Health Law, Education Law and New York Codes Rules and Regulations for health and education address immunization requirements for children in New York State Schools.

The minimum immunizations required for school attendance are:

3 Diphtheria (DTP, DPT, DT, DtaP OR DTB/Hib)
3 Polio (OPV) or 4 Polio (IPV)
1 Mumps administered after 12  months of age
1 Rubella administered after 12 months of age
2 Measles, the first administered after 12 months of age and the second after 15 months of age
3 Hepatitis B

All children born on or after January 1, 1998 and who are entering Kindergarten need one dose of Varicella.

Playground Information

Well our new playground structure is a big success with the children!  I am only requesting that you please have your child wear socks and sneakers.  This is safer on both the playground structure and blacktop.  A lot of the children are wearing "flip-flops", sandals and wedge heels and they are a disaster!!!

Also, it's a great idea to put your child's name in jackets, sweaters, lunch boxes, and anything else they may bring to school so that it doesn't end up in the lost and found.  This way, you'll get everything back.
